The California Department of Aging (CDA) is soliciting proposals from local governments and private nonprofit organizations to provide social and health care management services to persons aged 65 and over, who are eligible for Medi-Cal and certifiable for nursing facility care. Services are provided so that individuals can remain safely at home. Specifically, this Request for Proposal (RFP) is to solicit a contractor to replace the Multipurpose Senior Services Program (MSSP) site currently contracted to the County of Santa Barbara, Public Health Department. The new MSSP site will assume responsibility for any clients remaining from the current provider’s contract (estimated to be at least 150 clients, possibly more) and continue to acquire and maintain a full caseload of 200 total client slots over a 21-month period ending June 30, 2010.
